MyChart@TVC is provided by The Vancouver Clinic (TVC) to its patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully:

Enrollment
Use of MyChart@TVC is voluntary and enrollment is not required to receive care at TVC. Patients must be 13 years or older to enroll in MyChart@TVC. See below for information on parental access to a child's medical information.

Privacy
Your privacy is important to us. Please review TVC's Notice of Privacy Practices (https://tvc.org/notice-of-privacy-practices) for a thorough description of how we gather, use and protect your confidential medical information. All alerts sent within MyChart@TVC are subject to state and federal laws governing the security and confidentiality of protected health information. All of your health information will remain in our secure database and you will access it via the internet through our secure connection.

Access Code
During initial enrollment in MyChart@TVC, patients are given a unique access code. The access code allows the patient to create a personal login name (MyChart@TVC ID) and a password. Once this access code has been used to create an account it will no longer be valid. Patients are responsible for selecting a confidential password, keeping their password secure (not sharing it with anyone), and changing their password if there is suspicion that the password has been compromised. Patients may change their password at any time.

Accounts and Passwords
MyChart accounts and passwords may be used only by the authorized user. Passwords or accounts should never be shared with anyone, including trusted friends or family members. The Vancouver Clinic cannot be responsible for inadvertent disclosure of Protected Health Information if an account holder shares their login/password.

Password complexity requirements are: A minimum of 6 and up to 20 characters and must include letters and numbers (alpha/numeric).

Email Privacy
Patients electing to use MyChart@TVC must have an email address. Patients are responsible for acquiring and maintaining their email address and account. Patients with MyChart@TVC accounts will be notified via email when there is new medical information to be viewed via MyChart@TVC. This means that any person with access to a patient's email will be able to see these notifications. Although no protected health information will ever be sent via email patients should take this into consideration when providing an email address.

Time Sensitive Communication
MyChart@TVC may not be used for time sensitive communication. Patients requiring emergency assistance should call 911.

Integrity of Information
Information contained on MyChart@TVC may be changed or updated without notice.

Information Does Not Constitute Medical Advice
Information obtained from the MyChart@TVC does not constitute medical advice and should only be used for informational purposes. Please consult with a physician for proper interpretation of the information presented. Never change or stop any course of prescribed treatment without first consulting a physician.

Termination of Access
The Vancouver Clinic may restrict or terminate access to MyChart@TVC at any time for any reason.

Restrictions on Use and Ownership
All pages within MyChart@TVC and any material made available for download are the property of TVC. The content on MyChart@TVC is provided by TVC for its patients only. Nothing on MyChart@TVC should be construed as granting any license or right to use any trademark, service mark, or logo displayed on MyChart@TVC without the express permission of TVC or the third party that may own the trademark, service mark, or logo.

Supplemental Terms and Conditions for Parents or Legal Guardians of Children under the Age of 13

Parental Access
Parents or guardians of minor children may request access to a minor child's account. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient, age 13 or older, may not view without the consent of the minor. Because of these requirements when a minor patient reaches age 13 parental access is automatically terminated.

Parental access will only be granted to parties with parental rights over the medical record account holder (child). If the parent's legal relationship with the child changes, the parent must inform TVC immediately by calling 360-882-2778. TVC reserves the right to revoke parental access at any time for any reason.

Parental Termination of Access
TVC may, at its sole discretion, deny or revoke access for any reason including cases where parent/parent or parent/child disputes cannot be resolved. TVC may also revoke a parent's access if the child notifies TVC of verifiable emancipated status.

World Wide Access LimitedDue to the constantly changing cyber security landscape MyChart@TVC may not be available from some countries outside the United States.
